Dr. Richard Collins
Maple Grove Clinic

Dear Dr. Collins,

I am updating you about Mrs. Claire Wilson, a 71-year-old retired teacher, who has been attending physiotherapy following an ischemic stroke three months ago.

On initial assessment, she presented with left-sided weakness, impaired balance, and reliance on assistance for transfers. Over the course of ten weeks of physiotherapy, her program included strengthening exercises, gait training, and balance re-education.

She has made good progress, now able to walk short distances safely with a cane and manage transfers independently. Her endurance has also improved.

I recommend that she continue a community physiotherapy program for ongoing support and engage in light walking daily. Kindly review her during routine follow-up.

Yours sincerely,
