Dear Speech Pathologist,

I am referring Mrs. Hannah Green, a 54-year-old woman with progressive multiple sclerosis, for assessment and management of swallowing and speech difficulties.

She experiences difficulty swallowing liquids, occasional coughing and choking, reduced vocal clarity, and fatigue during conversation. These symptoms are impacting her social participation and confidence. Examination revealed mild tongue weakness, impaired coordination of swallow, and spastic dysarthria. She is currently on Interferon therapy and medically stable.

We request a comprehensive assessment of swallowing function and speech intelligibility, followed by therapy and dietary recommendations for a safe diet. Guidance for compensatory strategies and home practice to enhance communication efficiency and safety during meals would be appreciated.
