Dear Dr. Shah,

I am writing to report the lumbar spine radiographs for your patient, Mr. Robert White, a 58-year-old man with chronic low back pain.

The images demonstrate reduced intervertebral disc height at L4–L5 and L5–S1 levels, with anterior osteophyte formation and endplate sclerosis. Facet joint arthropathy is evident, most marked at the lower lumbar levels. No acute fracture, dislocation, or destructive bony lesion is observed.

Clinically, Mr. White reports chronic pain aggravated by prolonged sitting and morning stiffness. He also has a history of obesity and osteoarthritis. No neurological deficits were identified on examination.

The radiographic findings are consistent with degenerative spondylosis and facet joint arthropathy, correlating with his symptoms. Conservative management, physiotherapy, and lifestyle modifications may be appropriate, with further imaging if symptoms persist or worsen.

Yours sincerely,
