Dear Dr. Smith,

I am writing to provide radiographic findings for Mr. Alan Parker, a 42-year-old warehouse supervisor referred from the Emergency Department after sustaining a fall from a ladder earlier today.

Plain radiographs of the left humerus demonstrated a transverse mid-shaft fracture with moderate displacement and angulation. No joint involvement or dislocation was identified. Surrounding soft tissue swelling is evident, but the radial alignment is otherwise preserved.

Clinically, Mr. Parker reported severe pain and inability to move his arm following the fall. Examination confirmed deformity, swelling, and tenderness at the fracture site. His radial pulse remains intact, and no neurological deficit was noted. His medical history is only significant for hypertension.

In summary, the imaging confirms a displaced mid-shaft humeral fracture. Orthopedic review and intervention are strongly advised to determine definitive management.
