Case Notes:

  • Patient: Mr. John Carter, 64 yrs, retired bus driver

  • Referral source: General Practitioner

  • Presenting issues: Fever, productive cough with green sputum, shortness of breath ×1 week

  • Past medical history: COPD (diagnosed 5 yrs ago), type 2 diabetes, hypertension

  • Smoking history: 30 pack years, quit 10 years ago

  • Exam findings: Febrile (38.5°C), tachypnoea, reduced breath sounds in the right lower zone, coarse crackles

  • Social history: Lives with wife, independent in daily activities

  • Request: Chest X-ray to investigate possible pneumonia or COPD exacerbation

Dear Dr. Brown,

I am writing to report chest X-ray findings for your patient, Mr. John Carter, a 64-year-old man with fever, cough, and breathlessness.

The radiographs show a right lower lobe consolidation with air bronchograms, consistent with infective pneumonia. The lung fields otherwise demonstrate hyperinflation and diaphragmatic flattening in keeping with his known COPD. No pleural effusion or cavitation is identified. Cardiac silhouette and mediastinum are within normal limits.

Clinically, Mr. Carter has had a productive cough, fever, and breathlessness for one week. His history of COPD and diabetes increases his risk of infectious complications. He remains independent but has reduced exercise tolerance.

The imaging findings support a diagnosis of right lower lobe pneumonia superimposed on chronic COPD changes. Further management with antibiotics and monitoring of respiratory status is advised.
