Dear Dr. Morgan,

I am writing regarding Miss Emily Parker, a 17-year-old student who presented with a painful ingrown toenail affecting her right great toe.

The condition began two weeks ago and has worsened despite conservative measures, including warm water soaks and topical antiseptic cream. On examination, the toe is swollen and erythematous with localized tenderness and a small amount of purulent discharge.

Emily reports increasing pain, which interferes with walking and participation in basketball. She regularly wears tight footwear, which may have contributed to the problem. Her general health is otherwise good, with no relevant medical history.

Given the persistence of symptoms and early signs of infection, I recommend referral for partial nail avulsion. In addition, consideration may be given to antibiotic therapy should the infection fail to resolve promptly.

This intervention should help alleviate pain, prevent recurrence, and enable her to return to her regular physical activity.
