Dear Dr. Miller,

Re: Referral of Mr. John Walker - Recurring Plantar Fasciitis Management

I hope this message finds you well. I am writing to introduce you to Mr. John Walker, a 58-year-old teacher who has been referred to our clinic for the evaluation and treatment of recurring plantar fasciitis in his left foot.

Mr. Walker has been experiencing recurring episodes of plantar fasciitis in his left foot, which have been causing him significant discomfort and impacting his daily life, including his ability to perform his teaching duties. His symptoms include sharp pain and stiffness in the left heel, particularly in the morning, as well as increased pain with prolonged standing or walking. He has also noted decreased weight-bearing on the left foot and difficulties with teaching and other daily activities. On physical examination, I observed tenderness and swelling in the left heel, reduced range of motion in the left ankle and foot, and increased pain on dorsiflexion of the left foot. Notably, Tinel's sign was negative, and no neurological deficits were identified.

Based on our assessment, I have diagnosed Mr. Walker with recurring plantar fasciitis in his left foot. I believe that your expertise in podiatry will be invaluable in providing him with the care and support he needs to alleviate his symptoms and regain his comfort and mobility.

To address his condition effectively, I have developed an individualized treatment plan. This plan includes strategies for pain management, stretching exercises to improve flexibility, orthotic prescription to provide additional support, and education on appropriate footwear and activity modification to prevent future flare-ups.

I have every confidence in your ability to guide Mr. Walker toward relief and recovery. Please do not hesitate to reach out to me if you require any further information or clarification regarding his condition and treatment plan.

Thank you for your dedication to our patients' well-being.

Yours Sincerely,

Podiatrist
